Transportation Driver/Worker Waiver

I hereby release, waive, discharge and hold harmless First Baptist Church of Plattsmouth and its staff and volunteers from any and all liability, costs and expenses whatsoever arising out of or related to any loss, damage, sickness, or injury that may be sustained while attending and participating in activities associated with First Baptist Church. This agreement includes me, my children, my family members, and those to whom I am acting as guardian.
